2. PRINT OUT & MAIL: If you have a printer, print out this Registration Form as shown and provide the appropriate information. Address to:

3. HANDWRITE & MAIL: If you don't have a printer, write out your application, using the Registration Form as a guideline. Send to the address above.

Please give us as much of the information we're asking for based on your membership classification, up to your personal comfort limits!

YOU WILL NOTICE DOWN NEAR THE BOTTOM OF PAGE 2 OF THE REGISTRATION FORM THERE IS A PLACE FOR YOU AS A 'GENERAL SURVIVOR' OR 'PUBLIC INTEREST' MEMBER TO ENTER A VOLUNTARY (OPTIONAL) CONTRIBUTION AMOUNT.

PLEASE UNDERSTAND, THIS IS NOT A MANDATORY REQUIREMENT TO JOIN COSA,i AS A 'GENERAL SURVIVOR' OR 'PUBLIC INTEREST' MEMBER!

YOU DO NOT HAVE TO SEND IN ANY MONETARY CONTRIBUTION WITH YOUR APPLICATION TO BECOME A LIFETIME FULL PRIVILEGE 'GENERAL SURVIVOR' OR 'PUBLIC INTEREST' MEMBER OF THE CARBON MONOXIDE SURVIVORS ASSOCIATION, international, (COSA,i)!!!

We are only asking for financial assistance from those of you who are completely convinced as to the potential viability of this association and who can afford to send us a contribution (the wallet of your MD is starting to get low!). For those who do send a contribution - the full amount received will be credited to your account with COSA,i. Applicants for associate, academia and, other memberships are asked to fill in the appropriate "areas" on the registration form. You will be contacted by COSA,i to determine your dues/fees rates. Any contributions sent in with your registration will be credited to your account with COSA,i and will be used to offset your annual dues as determined per our follow up mutual agreement(s).

FOR THOSE OF YOU WHO DECIDE NOT TO JOIN OUR ASSOCIATION NOW PLEASE KNOW OUR DOORS WILL ALWAYS BE OPEN TO YOU IF YOU SHOULD FEEL AT A LATER DATE WE MIGHT HAVE SOMETHING TO OFFER YOU.
